All URG applications and the Professional Outcomes form have been moved online to Formstack. There is now one single Formstack application for First-Year, Research Fellowship, and External Grant Development URGs. There is a separate Formstack application for Small Grant URGs. The links to both applications can be found below.
Every application or form that previously required the signature of your Chair/Director will be routed to them for approval once you submit it via Formstack. We are no longer accepting paper applications or outcomes forms. If you have any questions or have any issues with the Formstack applications, please contact Jennifer Dodson at firstname.lastname@example.org.
In place of face-to-face workshops to assist with URG submissions, please review the URG Training PowerPoint for explanations of each type of URG along with suggestions on submissions. Examples provided below.
